    - Case Study Catastrophes

        Turbine Startup
                The facility was installing a steam driven turbine. This was a new installation planning to drive an electrical generator for electrical power production.

    During the construction phase the turbine was fitted with several automatic sensing alarm functions built into the steam and machinery operations system. To achieve better safety and be able to monitor the operational characteristics of the turbine. The decision was made to install a monitoring system on the physical measurements for locating the turbine position.

    During this installation the turbine was uncoupled from the generator. The personnel who were installing the monitoring system required that the turbine be operating in order to set their measurements. Attempting to start the turbine resulted in the system alarms for the machine sounding. As the turbine was not running the decision was made to isolate the machine alarms and start the machine.

    The turbine's inlet steam valve was opened and the rotor began to come up to speed. The bearing housings immediately began becoming very hot and the machine physically stop moving almost instantaneously.

    What happened?  This machine had catastrophic failure before every being placed into service. The bearing fits were scarred on the rotor. The bearing housings were destroyed and the bearing inserts were completely melted.  The machine required that the case be opened and all the internal components inspected, the rotor repaired and the total system inspected and refitted.

    The analysis for the root cause of what created this catastrophe.
    The steam turbine had an energy source supplied without basic inspection for the required system operations. The lubrication system for the generator and the turbine were secured and no oil pressure was present. The bearings had no lubrication and the turbine shaft was running on the babit faces of the bearings.

    What was the cost for this oversight? Lost production, delayed start up of the facility and depreciation of the physical components of the turbine.
